    Please complete this form to request the Headteacher of your child’s school to authorise a leave of absence during term time. There is no entitlement for any family holiday or leave of absence; therefore the Headteacher will only consider the request in exceptional circumstances. Our school is following Southampton City Council’s commitment to raising attendance and will robustly challenge unauthorised absence.

  • Administration of Medicine

    The school does not hold a supply of pain relief. Parents may supply medication for their child but we can only accept medicines if these are in-date, labelled, provided in the original container as dispensed by a pharmacist and include instructions for administration, dosage and storage. Please speak to Mrs Graves if you have any queries.

  • Confidential Medical Form

    Please complete this form if your child is going out on a school trip. Also, if there are any medical changes that the school needs to be aware of.
